  1. 1.     Evaluate/assess health care needs of the patient and make recommendations for admission into agency services.
  2. 2.     Admits patients for services in home health, early intervention team and other special programs.
  3. 3.     Develop and implement the plan of care for each patient, in cooperation with the patient’s physician by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests.  Address the patient’s specific needs, home situation and family/household unit.
  4. 4.     Reevaluate and record the patient care needs of the acutely ill and/or chronically ill at each home visit, noting all treatment modalities and frequencies.
  5. 5.     Communicate with and regularly update the patient regarding his/her plan of care.
  6. 6.     Provide skilled therapy services, treatment and procedures as appropriate for the patient’s care and safety, in compliance with productivity standards.
  7. 7.     Adhere to minimum productivity requirements as defined by Florida Home Bound MHA, Inc.
  8. 8.     Provide case management for assigned primary care patients as appropriate.
  9. 9.     Report observations of the patient’s condition to the designated supervisor(s) and/or the patient’s physician as appropriate.
  10. 10.     Treat clients to relieve pain and/or develop or restore function to maintain maximum performance.
  11. 11.     Provide skilled teaching to the patient and family members regarding the use of prosthetic, orthotic, and assisting devices (i.e., canes, walkers, wheelchairs, etc.).
  12. 12.     Orient and instruct Physical Therapy Assistants in the provision of care and services.
  13. 13.     Supervise the Physical Therapy Assistants at least once per month.
  14. 14.     Collaborate with other service providers to discuss the patient plan of care and frequency of visits.
  15. 15.     Make arrangements for outpatient services that cannot be given in the home.
  16. 16.     Participate in providing 24-hour, seven-day a week therapy services as assigned, to meet individual patient/family needs.
  17. 17.     Plan for client’s discharge, prepare discharge summaries and instructions.
  18. 18.     Drive safely, comply with traffic regulations, and wear seat belt when business travel is required.
  19. 19.     When appropriate, supervise Home Health Aides.
  20. 20.     React to change productively and perform other job related tasks and duties as assigned.
  21. 21.     Serve as a mentor for other agency personnel and/or students engaged in clinical affiliations.
  22. 22.     Act as a role model in confidentiality, professionalism, appropriate attire, unbiased interpersonal relationships and attitudes.
  23. 23.     Comply with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.
  24. 24.     Ensure confidentiality of patient records/information.
  25. 25.     Attend team meetings and staff meetings to enhance team communication, coordination of services, and quality services.
  26. 26.     Implement a multi disciplinary approach to patient care by assessing and identifying the patient’s need for other agency services and communicating that need to the designated Team Leader and/or the patient’s physician.
  27. 27.     Participate in interdisciplinary case conferences and in-service education programs as required by agency policies and patient needs.
  28. 28.     Participate in performance improvement activities.
  29. 29.     Generate re-certification of the physical plan of care, ensuring timeliness and quality of documentation on a timely basis.
  30. 30.     Record clinical and progress notes at the time each of visit to ensure continuity of qualitative patient care on a timely basis.
  31. 31.     Observe, record and report (1) signs and symptoms, (2) reactions to treatments and medications, and (3) any change in the patient’s physical or emotional condition on a timely basis.
  32. 32.     Complete daily report sheets and any required paperwork in a timely fashion to ensure agency operating schedules.
