INITIAL/BI-ANNUAL MANDATORY ORIENTATION

CNA/HHA

Information For Home Visits
A Supervisor must monitor the quality of all Home Health Services. This supervision is mandatory for Aides/HHA, PTA and COTA. We will hereby request that patients consent to have state/accreditation companies/federal health survey personnel conduct a “home visit” to ensure that the federal/state requirements are met and to assist in evaluating the effectiveness and quality of home health services that are received by us. The patient’s consent is voluntary and no rights to confidentiality or privacy are waived by any consent. Patients must be told and understand that refusal to consent to a home health visit will have no effect on the level or nature of Medicare/Medicaid/Insurance benefits to which they are entitled.
